| | Claim & Emergency Medical Evacuation Procedure For Corporate Business TravelImportant Information Call your insurance broker/agent for a copy of Claim Form; - The Claim Form has to be completed, signed and submitted together with the original supporting documents according to the section of loss stated below;
- All Claims Notification must be sent to American Home Assurance (AHA) in Singapore within 30 days after the occurrence of any event which may give rise to a claim. Notification could be verbal or written, followed by the completed claim form;
- To enable the Claims Department to process the claim at its best, the following information should be provided at the time of notification:-
(a) Policy Number (b) Circumstances of the accident/loss (c) Date/Place of accident/loss (d) Extent of loss (e) The type of coverage provided
Section of Loss- Accidental Death and Compassionate Death Allowance
Obtain a Death claim form from American Home Assurance (AHA). Upon completion and duly signed, return the form together with the following enclosures:- a) Police Report b) Death Certificate c) Supporting proof such as autopsy report, newspaper, if any d) Air Ticket and a Copy of Passport with Boarding Stamp - Permanent Disablement and Fracture Benefit
Obtain a Personal Accident claim form from AHA. Upon completion and duly signed, return the form together with the following enclosures:- a) Medical Report b) Police Report c) A detailed description of how the accident occurred and the injuries sustained d) Air Ticket and a Copy of Passport with Boarding Stamp - Medical & Accidental Dental Expenses, Return Treatment and Treatment by TCM
Documents to enclose include:- a) Original medical bills b) Medical Report c) Air Ticket and a Copy of Passport with Boarding Stamp - Hospital Confinement Benefits
Documents to enclose include:- a) Original supporting documents from the hospital whilst overseas stating the period of hospital confinement due to sickness or accidental injury - Emergency Medical Evacuation, Repatriation Expenses and Direct Repatriation Expenses
Emergency Medical Evacuation and Repatriation will be organized by American International Assistance Service, Inc (AIAS). AIAS will contact AHA Claims Dept for matters pertaining to Emergency Evacuation or Repatriation. Covered expenses will be paid directly to AIAS by AHA, subject to the policy terms and conditions.

Documents to enclose include:- a) Medical Report b) Transportation Bills (Air Ticket or Railway Ticket) c) Hotel Bills For Accommodation Only d) Air Ticket and a Copy of Passport with Boarding Stamp - Trip Cancellation
Documents to enclose include:- a) Death certificate / Doctor’s letter b) Invoice from the agency and statement indicating the refund c) Invoice showing the cancellation charges charged by travel agency d) Proof of relationship (Birth certificate and marriage certificate) e) Air Ticket and a Copy of Passport with Boarding Stamp - Trip Curtailment
Documents to enclose include:- a) Additional ticket purchase b) Letter from the agency which shows the breakdown of the trip that has not been utilized c) Doctor’s letter/Death certificate d) Proof of relationship e) Hotel statement for accommodation paid in advance (for free-and-easy schedule) f) A Copy of Passport with Boarding Stamp - Travel Delay
Documents to enclose include:- (a) Travel itinerary (b) Travel documents such as boarding pass/tickets indicating actual departure time & date (c) Written confirmation from carriers/their handling agents specifying the reason and hours of delay. (d) Air Ticket and a Copy of Passport with Boarding Stamp
Remarks: Departure point must be outside the place of origin of the trip - Travel Misconnection
Documents to enclose include:- (a) Travel itinerary (b) Travel documents such as boarding pass/tickets indicating actual take-off time & date of the onward travel connection (c) Written confirmation from common carriers/their handling agents on the incoming delay of scheduled public conveyance at the transfer point (d) Original bills/receipts identifying expenses incurred for hotel accommodation/meals/refreshments e) Air Ticket and a Copy of Passport with Boarding Stamp
Remarks: Transfer point must be outside the place of origin of the trip - Baggage Delay
Documents to enclose include:- (a) Travel itinerary (b) Written confirmation from carrier specifying the reason of delay (c) Receipt of acknowledgement on returned baggage (d) Air Ticket and a Copy of Passport with Boarding Stamp
Remarks: Delay must be outside the place of origin of the trip - Damage or Loss of Baggage and Personal Portable Business Equipment
Documents to enclose include:- a) Property irregularity report if baggage is lost or damaged by carrier or police report b) Details of items lost/damaged including date of purchase/actual cash paid, year of make. Attach receipts of items lost, if any. c) Air Ticket and a Copy of Boarding Stamp
Remarks: Any loss must be reported to the police within 24 hours - Loss of Money Loss & Travel Documents
Documents to enclose include:- a) Police report b) Receipts of the replacement passport c) Hotel bills for accommodation only due to delay from replacement document. d) Transportation bills e) Administration charges for obtaining a replacement air ticket
Remarks: Any loss must be reported to the police within 24 hours - Legal Fees
Documents to enclose include:- a) Police Report b) Legal documents inclusive of invoices and reports against 3rd party claims. - Personal Liability
Do not admit liability or make any offer, promise or payment without prior consent. Submit all correspondence/documents from third parties for our handling. - Credit Card Indemnity
Documents to enclose include:- a) Police Report b) Medical Report if the insured person suffers from permanent disability. c) Death Certificate if the insured person has passed away. d) Credit Card Monthly Statement - Child Education Fund
Documents to enclose include:- a) Police Report b) Certificate of Birth c) Official receipts from the education institution - Hijacking
Enclosures include written confirmation such as police report or report issued by the common carrier that the Insured Person was a victim of the hijack and the duration of hijack.
